During your first visit, we will review your dental and medical history forms, and take X-rays of your teeth and mouth.

Your first visit is all about you -

your comfort, your happiness, and your health.

Then, the doctor will meet with you to discuss your X-rays, help you create your own individual treatment plan, and answer any questions that you may have.

At your next visit, one of our hygienists will clean and polish your teeth, checking your teeth and gums for any cavities or early signs of tooth decay or gingivitis.

Bring your smile, but also remember to bring these with you to your first appointment:

For your convenience, we have made all our patient forms available online! If you are a new patient, you can download our patient forms or print them. Please fill them out and bring them with you to your appointment. If you are an existing patient and need to update your information, please call us and we can update your file.

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, treatment can be done the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day. Please assist us by providing the following information at the time of your consultation if possible:
